Abstract

Objective:  Verify if the patients who received discharge prescriptions containing enoxaparin in the orthopedics and surgical clinic sectors of a large general hospital in the city of São Paulo received pharmaceutical orientation and its influence on treatment adherence.

Methodology: This is a cross-sectional, retrospective, and quantitative study. Data collection was carried out using electronic medical records. Patients who were prescribed enoxaparin at the time of discharge were selected and the record of pharmaceutical guidance was verified. In a second phase, it was verified whether these patients took their medication from the health system. Results: Patients who need to extend venous thromboembolism prophylaxis after hospital discharge use enoxaparin, an anticoagulant for subcutaneous application. 50 patients with enoxaparin prescription at discharge were analyzed. 52% of the records stated that patients received pharmaceutical advice. Of the total number of patients, 76% took the medication from the healthcare system and only 6% returned due to an adverse reaction, or because they did not know how to use the medication, or because they developed VTE. Conclusion: Patients who received pharmaceutical guidance at hospital discharge did not report VTE or have any doubts about the treatment. It could be because there was better adherence to extended prophylaxis in this group because most of the patients took the medication from SUS pharmacies. Pharmacotherapeutic monitoring and discharge guidance by the clinical pharmacist for patients who require postoperative thromboprophylaxis actively contribute to patient safety and treatment effectiveness.
