Objective: To describe and analyze premature mortality (PM) of elderly people due to chronic non-communicable diseases (NCDs) in Brazil, in the State of São Paulo (ESP) and Municipality of São Paulo (MSP). Method: Quantitative, descriptive and cross-sectional study carried out using the System of Policy Indicators and Monitoring of Policies for the Elderly (SISAP-Idoso). Data on PM of elderly people were extracted for four NCDs in the period from 2000 to 2019, by sex and location, grouped into tables and statistical tests were applied to analyze the data. Results: PM due to cardiovascular disease and neoplasia were higher than PM due to chronic respiratory disease and diabetes mellitus regardless of location (p<0.001). Analysis by sex revealed that PM for elderly men is significantly higher than that for women, regardless of location (p<0.001). Conclusion: Even with the efforts of Primary Health Care, PM rates for NCDs are high. Health promotion and prevention actions should be emphasized by the Family Health Strategy, considering the specificities and needs of the population.
